The groundbreaking Pediatric Intensive Case Management Program at NAM in partnership with The Episcopal Health Foundation helps children like Angel  manage chronic conditions. Angel suffers from asthma and other ailments that hinder his quality of life, but since his assessment at the Clinic and thanks to our health partners, Angel’s health has dramatically improved and he’s now making great strides toward living a happier and healthier life.

The Pediatric Intensive Case Management Program deals with conditions like asthma, diabetes, obesity and others, and it offers a holistic approach that allows the Clinic’s medical staff and families to assess, plan, implement, coordinate, monitor and evaluate the options and services to treat chronic illnesses in a way that meets the individual’s health needs and that of their families.
